1. Serves as a liaison between the recovery community, the addiction treatment system, the medical treatment system, and the patient’s community, family, and social context to facilitate connections across systems of care.
2. Works collaboratively with treatment team in support of evidence-based services to patients with addiction disorders.
3. Works with patients and their providers to assess readiness for behavioral change, level of intervention needed, and engages patients in development of collaborative treatment plans to achieve goals of risk reduction and/or abstinence.
4. Addresses barriers to successful recovery, works collaboratively to remove barriers and serves as role model and advocate
5. Conducts active outreach efforts to encourage attendance and participation in recovery-oriented, self-help, and pro-social groups.
6. Assists with access to treatment for addiction disorders and co-occurring conditions, creating systems and procedures to rapidly identify treatment needs and determine appropriate level of care.
7. Assists with data collection for quality assurance and program evaluation
Help patient navigate the healthcare system and social service system; accompany patients to appointments and meetings as needed.
8. Negotiate and create opportunities for access to treatment and social services for patients; advocate on behalf of patients to help decrease barriers to care
9. Educate patients about addiction and utilize motivational enhancement techniques to explore ambiguity and tip the scales towards treatment
10. Provide peer coaching and support patients in establishing treatment goals and achieving and maintaining recovery.
11. Develop wellness plans for each client with advice from the Clinical team.
12. Work with the care team and patient to support and advocate for the patient during the development of a comprehensive care plan
13. Be available to provide support to patients in immediate risk of relapse or for other time-sensitive matters during and outside of regular business hours
14. Monitor patients’ progress and evaluate outcome, using a patient tracking system
15. Clearly and effectively facilitate communication between the patient, PCP, and Addiction Medicine provider.
