Position Summary:
The Case Manager provides compassionate, quality clinical care under the supervision of a licensed physician. The role involves assisting the aesthetic and wellness physician in all patient care, performing assessment and implementing treatment plans for patients seeking aesthetic and preventive healthcare services. Additionally, the role involves educating patients about treatments and wellness practices while adhering to clinic policies and regulations.
Responsibilities
Obtain and review comprehensive medical histories, physical data, and existing patient records.
Build strong rapport with patients by understanding their needs and exceeding expectations.
Perform patient assessments through interviews, chart reviews, and physical exams, considering medical, psychosocial, and cultural factors.
Order and interpret diagnostic tests and screenings ordered by physician.
Educate patients and families on medications, treatment plans, and preventive healthcare, including disease management and wellness strategies.
Document patient examinations, results, diagnoses, and treatments in electronic medical records.
Ensure regular and effective communication with physicians regarding patient status, especially significant or unusual findings.
Collaborate with physicians and multidisciplinary team members to manage both acute and long-term care.
Ensure continuity of care between visits by triaging patient calls and managing follow-ups.
Recommend community resources to meet patient and family needs.
Maintain patient confidentiality in compliance with clinic policies and regulations.
Additional Responsibilities:
Participate in required meetings, training sessions, and professional development activities.
Assist with the efficient patient flow and coordination of resources, working closely with site managers and physicians.
Engage in professional development, including continuing education and maintaining affiliations.
Participate in peer reviews, quality assurance activities, and staff education to ensure compliance with standards and policies.
Collaborate with the healthcare team on patient advocacy and overall well-being, including mental and emotional health.
Generate sales with collaboration with a medical team.
Job Requirements:
Proven work experience as a Case Manager or Physician Assistant or Nurse or related field.
Strong knowledge of medical guidelines and aesthetic treatment protocols.
Ability to assess patient status, develop treatment plans, and provide education, in accordance and under physician care.
Excellent communication and patient rapport-building skills.
Commitment to patient confidentiality and high ethical standards.
Ability to collaborate effectively with physicians and healthcare teams.
Ability to work in a fast-paced environment and handle multiple tasks.
Strong attention to detail and problem-solving abilities.
