Job Responsibilities:
Ensure all prescriptions are accurately dispensed to the correct patient at all times:
Check all medications supplied are according to the prescription with accurate labels and correct physical medications.
Review appropriateness of prescription and verify with prescriber if necessary.
Seek advice from pharmacist where required.
Counsel patient on proper medication administration & technique (where applicable) and advise on appropriate storage condition.
Provide alternatives or suggestions on medication treatment where appropriate.
Provide basic medicine information e.g.
indication, dosage, adverse effects, method of administration, etc.
Ensure medications are stored according to recommended conditions to maintain efficacy and quality:
Adhere to recommended storage requirements when handling medication in the pharmacy at all times.
Accurate documentation to ensure compliance to regulatory requirements and accreditation standards:
Document all daily transactions completely and accurately in the required manner e.g.
Poison Register, Dangerous Drug Register, Psychotropic Drug Register, Codeine & Pseudoephedrine Register, in accordance to regulatory requirements and accreditation standards.
Ensure quality and sterility of medication:
Adhere and prepare formulation in accordance to Good Manufacturing Practice e.g.
wearing of Personal Protective Equipment during manufacturing.
Perform cytotoxic drug reconstitution using aseptic technique in accordance to procedure.
Ensure all ward stocks are accurately supplied to wards and departments:
Prepare and supply all ward stocks according to requirements.
Ensure standards & customers' expectations are met.
Provide general administrative support within Pharmacy Department:
Accurate documentation to ensure compliance to regulatory requirements and accreditation standards.
Ensure proper documentation in accordance to business, statutory and accreditation requirements.
Job Requirements:
Diploma in Pharmacy from a recognized institution, or certificate of dispensing/relevant certificate from a recognized institution.
Experience in hospital pharmacy is an advantage, preferably with 2 years of working experience in the hospital industry.
Able to work shift and in a fast-paced healthcare environment.
