Social Worker/ Case Manager

The social worker/case manager will be responsible for assigning, monitoring, and organizing patient cases.
They will be directly involved with their patients in providing resources and assistance.
Planning rehab and scheduling treatment for patients is all in a day's work.
The social worker/case manager must be highly organized, empathetic, compassionate, non-judgmental, and eager to help vulnerable adults.
KEY RESULT AREAS:
Follows policies and procedures with regard to patients records to ensure that the services provided are consistent with best practices, assessment, case management and/or treatment goals.
Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and patients.
Has a therapeutic approach of being empathetic, respectful, and motivational, while encouraging patients to take responsibility for their lives.
Demonstrates high professional and ethical standards.
Acts as a positive role model, providing patients with guidance and support.
Responsibilities
Assist in establishing, implementing and administering unit policies and procedures.Follow up on patient referral list and guarantee letter.
Updating of case/ referral status.
Prompt submission of case management report and other documents as required.
Request for renewal of Guarantee Letter.
Submission of progress report and discharge report.
Request for assistive/ adaptive equipment.Promotes IHT Rehab facilities and service provisions to the patients and family members.
Track and collate number of patients contacted, home visits and other data as required for reporting to management.
Accessing and developing strategies to assist in the patient needs.
Ensures patients' rights and maintaining patients' confidentiality at all times in accordance with PDPA standards.
Assumes primary responsibility for all case management functions of patients served, including monitoring through a comprehensive service plan, coordination and advocacy.
Supports patients to make decisions related to plan of care and recovery-oriented approach.
Facilitates awareness of life now and how they want it to be.
Facilitates a specialized group work using evidence-based practices.
Works with patients to identify and eliminate barriers from seeking treatment (i.e.
issues of food, housing, transportation); to allow greater participation in treatment as needed.
Ensures collateral contacts with other essential services are identified and documented within the plan of care.
Sets up transportation for patients to attend appointments.
Maintains and utilizes knowledge of community resources and accessibility.
Schedules appointments with consideration of patients' needs and preferences.
Refers patient served to other professionals or services as appropriate to ensure optimal service; monitors and follows-up on status of referrals and existing services.
Ensures that documentation is complete, accurate, timely and meets regulatory and standards.
Maintains appropriate boundaries and displays respect and courtesy to all internal and external customers.
Demonstrates patience, caring, compassion, and sensitivity toward patients.
Functions in a cost-effective manner with regard to the utilization of time, supplies and other rehab centers resources.
Attends meetings and supervisions as required and necessary for the position.
Promotes and fosters effective relations, both inside and outside of the rehab center.
